Trauma Surgery: what it involves

Orthopaedic trauma is time-sensitive. A fracture that is reduced and stabilised correctly in the first days heals in its proper shape; the same fracture left to unite crookedly can mean a lifetime of limping, stiffness or early arthritis. Our aim in every trauma case is anatomical alignment, stable fixation and early movement.

Orthopaedic trauma covers fractures of the hip, femur, tibia, ankle, wrist, forearm, humerus, clavicle and pelvis, along with joint dislocations and injuries involving tendons, ligaments and nerves. Depending on the injury, treatment may mean closed reduction and casting, intramedullary nailing, locking-plate fixation, external fixation for open injuries, or percutaneous fixation where a smaller incision will do the job.

Some patients come after being treated elsewhere, with a slipped fracture, a bone that has not healed, an infected implant, or a limb that has united at the wrong angle. These situations need honest assessment before anything is planned, because the second operation is usually harder than the first.

Recovery

Most simple fractures unite in six to twelve weeks, with heavier bones such as the femur and tibia taking three to six months to regain full load-bearing strength. Movement of the neighbouring joints usually begins within days of fixation, because stiffness is far harder to treat than the fracture itself.

Trauma Surgery: common questions

Do all fractures need an operation?
No. Many fractures that are in good position and stable heal perfectly well in a cast or brace with regular X-ray checks. Surgery is advised when the fragments are displaced, the fracture involves a joint surface, the bone is unstable, or when fixation will let you move and return to work much sooner.
How soon after an accident should I be seen?
As soon as possible. Open wounds over a fracture, a cold or numb limb, or an obvious deformity are emergencies. For closed injuries, being assessed within the first few days gives the widest choice of treatment and the best final alignment.
My fracture was operated elsewhere and has not healed. Can it be fixed?
In most cases, yes. Treatment may involve removing the old metalwork, clearing any infection, bone grafting and re-fixing the bone in correct alignment. What is possible depends on the state of the bone, the soft tissues and whether infection is present, so it needs assessment with fresh imaging before anything is planned.
Will the plates and screws need to be removed later?
Usually not. Implants are designed to stay permanently and cause no problem. Removal is considered when metalwork becomes prominent and painful, when it restricts a joint, or in children and young adults where growth or future surgery makes it sensible.
